On the very off chance that I need to make a map from a single image, Materialize does me good.
Shame the UI doesn't scale nice though.

I usually don't do that, but if i had to I would use Substance Designer since it has the bitmap2material node (which was once a separate stand-alone program).

Best quality normals from photosource I've achieved just using the Photoshop emboss filter, two 90degree horizontal/vertical aligned passes to create the XY, red and green channel.
blue Z channel I create with the highpass filter, changing levels as needed for a balanced midpoint and black/white balance.
I normalize the result using xNormal.
