you'll basically need to:

>Download the valve porting tools for blender, max or whatever software you're using
>Assign the weights, bone groups and jigglebones properly.
>Scale your model in a way that it ends up with the same scale as of a normal valve human model, in your case
>Export your model as DMX, since it's easier and it already compiles the face flexes
>Write a basic QC file
>Convert your textures to VTFs and drop them in a folder somewhere in your SFM main folder

Notice you cannot have places where you have more than two weights as source does not support that for some reason and you'll dissapoint yourself with innumerous rigging errors (if on garry's mod, you might get an extremely stiff ragdoll)

Adding to that, your model seems to be already rigged and faceposed, so it wont be much of a hassle. It doesnt need to have shapekeys or face bones but if it does it'll just make the porting process easier.
Also SFM porting seems to be much easier.
