How do u rig the wrist so when you rotate the hand the model doesnt become like it has been in meat grinder?

If you think about it a little bit the solution is obvious.
It's the same idea, but there's two approaches.
The simpler one is to have a twist bone, a bone between the elbow and the wrist that you axis lock to bone roll rotation, parented to the elbow bone, with the wrist parented to it, and have several hidden bones hanging off the elbow bone without anything parented to them that copy the twist bone's rotation at different influences.
The slightly more complex one is to also have hidden bones hanging off the elbow bone, but not have a twist bone and rather isolate the Y rotation (ie the bone roll axis) of the wrist bone and copy it at increasing influences to the hidden twist bones. That way the hidden twist bones are transformed last (ie after the wrist as well) but the result is good. This is more complex because you don't often get that much control over bone constraints in game engines, so you'd either have to figure out how do isolate the Y rotation manually or keyframe those hidden bones' transforms in animations (but then you couldn't have good wrist twisting dynamically).

Well, during the animation phase, animators do tend to use lots of helper bones because animating all bones just using FK is kinda silly given the tools at one's disposal. But games are all different and lots of them don't have any dynamic rigging at all so the result is often baked down to just deforming bones to save space. If you're asking if you're forced to "learn complex rigging" to make something passable then not really, you generally just need leg, and maybe arm IK and some arm+elbow twisting solution.
