/3/ Do you have any advice on how to photoscan (or scan another way, preferable cheaply) my head? I've tried this so many times, but all I've been able to create is this scan in the image..

You'll need around 70 high quality cameras on a rig and Gentoo
Or maybe look into structured light scanning but you'll need to buy a projector.

Buy a Kinect (those shitty devices for the X360/Xbone), they're actually really good for professional use on the cheap. The V2 ones have much better resolution but are more expensive.

I was able to scan my head with an old dlsr and agisoft. I had my wife take the photos and the results were far better than what you have posted so far and that was around 8 years ago.
Get someone else to take the pictures for you if you haven't done so yet.

I don't think those can do mm precision. You need enough megapixels so that each pixel represents a tiny tiny space on your face, otherwise you wont get the edges and creases
