Is there an actual 3D texture painting tool? Even stuff like Substance Painter or Quixel Mixer still use the UVs for most of their operations and just translate mesh space to UV space first, with only a few tools not doing so (e.g. drips etc).
Is there a tool out there that actually uses the mesh topology and only bakes out the result onto the UVs?

Ptex was developed for this, but it's only used in a few prosperity, internal studio tools. It never gained traction in consumer software.

blender can do this, its called vertex painting
zbrush can also do this and its called polypaint
3d coat has a workspace for it to just called "paint"
if you want a standalone application armourpaint is free and runs pretty smooth

You're just wrong OP, substance painter uses a point cloud to record stroke positions then generates the texture based off that, this means you can completely change the UVs, reimport the file and substance will just recalculate it.

That recalculation can still destructively alter the final look if the UVs have been modified significantly enough.
No, the only true 3D painting technique is one that records information on vertex/voxel data directly without the need for UVs

It uses some mesh data but still relies heavily on UVs. You can see that in baking maps where bad UVing will result in visible seams.

Not the same thing. That only colors the vertex itself and then the color interpolates between vertices along the face.
