How are textures like this made? Is it mostly just flat colors with gradients and baked lighting/AO to add more detail, along with some handpainted features like the bandages?

my best guess
1) model and UV unwrap. think about your UVs and stack them accordingly
2) create a colour mask. basically a texture map where each object is identified by different colours. can be baked in most 3d programs
3) go into photoshop and fill in your basic gradients and colours.
4) go into substance and start painting your details on the albedo channel. paint mainly on the 2D texture view for clean textures.
i don't think any baked lighting or AO was used. maybe hand painted but it never works that well especially with stacked textures (e.g. the ear)

Yeah, I'm guessing the ear was hand-painted, as were the ropes, bandages, and the patterns/folds on the clothing.
