How are folds like pic related modelled? Are they just sculpted + retopo'd? What if you wanted to do it manually and without baking? Proportional editing?

mask the bottom part (belly) => pull down the (non-masked) upper part => repeat for each fold.
eyelids, which most people struggle with, are done the same way.
