Is it possible to genetically modify a plant so that seeds and fruits are completely separated from each other? (see pic related)

Probably not. It would be easier to make two cultivars, one seedless and the other with tons of seeds, and graft them together, but the seeds wouldn't grow into the same plant. You'd have to graft the seedless variety onto each plant individually.
Your image looks more like a tomato than anything else. If you just want a tomato plant that lasts longer than a year then you could grow it inside. They're perennials in warm enough climates.
