Also, If you are talking about 1000 CE to Mughals, Well, They had pretty good idea of who was around. Otherwise Why would Chinese scholars come to india and also, Silk route and stuff. But yeah, Himalayas have always been hard to cross. So There was a little less mixing.

Also, If you talk about older than that, Like before 2500bc and stuff, Like the times of Ramayana. They had even better knowledge. If You read ramayana, There stori where Sugreev sends his Vanar Army in search of Sita, He tells them to go in all directions and come with any news in so and so days. We all know that part, But, Did you know, He actually tells to them about all the places in pretty much all details, Like when you go in North from central India, first this place will come then this, then this.
