They wouldn't annihilate, they would just merge into one bigger black hole. In a black hole described by GR a black hole has a singularity, which has an infinite density and so isn't composed of anything that could be called anti or normal matter. So it doesn't matter. Many people believe the singularity is not physical and some future theory will show that it has a finite density, in which case it's physics is unknown. But even if that matter could annihilate into light, the photons couldn't escape and the black hole would still be as massive as before.

Matter in orbit around a black hole ultimately falls in because of friction with other matter, right? If there was an isolate particle, in orbit inside the event horizon, it would never actually fall in toward the center, would it?

Nothing interesting. Black holes are not made of matter or antimatter. They're not things in the usual sense of the word. They're just regions of spacetime that are bounded by an event horizon which, due to the curvature of space in the region, looks spherical. Throw antimatter into a black hole and you just get a slightly larger black hole. No annihilation can occur because there's no stuff there.

Mass and energy are the same. You could make an artificial black hole by concentrating a critical amount of light at a single point and would be indistinguishable from a stellar black hole.

In fact the nearest stable orbit is OUTSIDE the event horizon, iirc ~1.5x for non-spinning black holes and closer for Kerr ones
