Every single case of "depression" I've seen boils down to one or more of lack of physical activity, poor diet, drugs, no hobbies, and no pussy.

Honestly OP, your brain changes as you get older. The shit that you obsess over when younger just doesn't matter as much as you get older. For better or worse.
Get the right diagnosis. Stuff like CPTSD can be hard as fuck to diagnose and lead your doctors down blind alleys for years. If you're not making progress with one diagnosis, get another opinion. You might have autism. You might have ADHD. Understanding your problems are crucial to getting better.
Check for physical ailments, Thyroid, Lyme disease, all types of treatable stuff can cause lifelong depression.
Nutrition is really important. Dehydration can cause severe fatigue and mind-fogginess. Check for gluten sensitivity. Make sure you get enough protein.
Physical exercise is hard but just walking a few minutes a day can help a lot.

You don't. There is no hope.
1. Depression is not something to cure, not an illness. It is a natural physiological reaction to external circumstances such as isolation, discrimination, exclusion, bullying, neglect etc. You're rightfully feeling helpless and hopeless.
2. After 20+ years this condition has restructured your brain, permanently scarred your personality. This damage is irreversible. At this point, even if all external circumstances changed instantly you wouldn't be able to live a healthy normal life.

If your T levels are fine and you genuinely have low libido and aren't doing the typical 4chins MGTOW tradman cope, then it could mean finding a hobby that gets you out of bed. I remember years ago when I was "depressed" and was already /fit/-maxxing, getting not-too-shabby results on dating apps, etc., I stumbled across a /sci/ book chart thread for STEM textbooks. Wanting to grind some more exercises is literally all it took to make me excited about getting out of bed. But probably most people will have to do something less autistic like learning an instrument, making videos, doing martial arts, or some such.

OP here, I have been diagnosed with clinical depression multiple times by different medical professionals. Tried SSRIs, ECT and TMS without much effect. Trust me bro I know what depression is. They apparently want literally drill into my skull to fix it with Deep Brain Stimulation.

Depression is related to lack of control/agency. The best way to counter that is to exert your own will, which is action. Any action would do to reduce depression, however meaningful actions lead to significant reduction of depression.
Minor things like writing, drawing, exercising, morning jogs, cooking, cleaning reduces depression. Major big achievements work a lot in reducing a lot of depressions. Like working on a big art project, finishing that special hobby that you're working on, achieving some important goal like losing weight/gaining muscle mass/etc.
