It happens beyond the epistemological event horizon. The collapse of the wave function is non-informational. The no-communication theorem holds. Relativity isn't violated. You can view it in analogy to qualia which are also not translatable into information. (There is indeed a deep connection between consciousness and quantum mechanics but I won't elaborate because it makes simple minded people seethe.)

There is no single universal gate because SU(2) is not a cyclic group. Best we can do is an approximately universal set of generators of a dense subgroup such as Hadamard, CNOT and pi/8.
This is why no quantum computer will ever be able to compete with human consciousness. Consciousness doesn't need discrete approximations. Consciousness is inherently continuous and hyperturing.
