The only time where it isn't and very vulnerable is at the point of transmission, when it's still a foreign organism. It binds to your CD4 immune cells and rapidly propagates from there (this causes initial fever symptoms).

PreP binds to the receptors that HIV tries to bind to thereby blocking off its growth potential. This is the stage at which HIV can't grow anymore and your body's immune cells find the foreign organism and destroy it.
