Fairly high chance of infection, not necessarily from MRSA, but it is generally treatable if doctors *actually know what they are doing*.
If Trump's doctors are not shit-tier right now, they have him 100mg/day of zinc supplements as anti-sepsis prophylaxis, have taken a culture of the ear wound, and have him on some random antibiotics until the culture comes back.
