Anyone with a doctorate degree is a real doctor. If MDs want to be special, they should call themselves MD instead of using the generic term doctor. That probably would be better for everyone anyway since people with a DO, DC, or other degree related to medicine isn't confused with an MD. Also the medical profession lost its ability to claim some kind of special status in society after their behavior during the pandemic showed them to be mostly automatons blindly spitting out whatever they were told by a political authority figure.
