It depends how "deep" the anesthesia needs to be. If you're doing some messy surgery where the less "awake" they are the better, then, I'm guessing it's because there's excessive/unnecessary/undesirable side effects when used at high anesthetic levels, compared to other substances.
If I had to guess.

I've only read about kids doing Lean, I don't really know, haha.
