Opioid drugs are distributed everywhere, whereas endorphins can have a localised effect. An "endorphin rush" occurs when endorphins are released in the nucleus accumbens, which is responsible for opioid euphoria. Whereas opioids introduced into the body will bind to receptors across the brain, including the brainstem where they depress the peripheral nervous system (PNS), which can cause nausea. Also, the PNS depression from endorphins doesn't typically cause nausea, because the endorphins are released to numb an overactive PNS. The same is true for opioids in cases of severe pain and stress.
