Is this really a good healthcare policy decision? Seems like a bad idea

Increase organ pool.

Letting HIV+ organs be transplanted is insane. You're literally introducing a deadly virus into someone just because 'muh donor pool.' Instead of pushing this madness, maybe focus on actual prevention and better treatments. Stop normalizing unnecessary risks for the sake of woke medicine.

But then there's no procedure to get paid for.

It's HIV to HIV only, but bypassing the clinical research requirement and review doesn't sound like it "removes unnecessary barriers"

On organ failure? What the fuck are you actually talking about.
HIV hasn't been a "deadly" virus for decades now. The virus can be so controlled that it's undetectable and can't be spread by sex. If you had to choose between literal death and having to take some HIV pills for the rest of your life (which honestly isn't going to be too long if you're getting organ transplants) I think 99% of the people would just take the fucking pills.
This only makes sense if you're wait is so long that you could potentially die without an organ transplant though.
