its not the mass distribution, its the changing mass distribution.
the earth is not a rigid body like the tennis racket, all the water and to a lesser extend the earth itself sloshing around over billions of years that'll cancel out the secondary axis.
You don't get the tennis racket effect with a spinning water bottle or any other non-rigit body with dynamic mass distribution on the ISS.
