smooth preview is not reality. It's an illusion and sometimes it has errors like this. Turn it off and hit it with the actual smooth tool. If it's still there then you have a problem in your mesh you have to fix. If it disappears go back and just ignore it. It will eventually disappear as you add more geometry.
